AED offers several scholarship
applications for Chapter Member:.
Please read through these guidelines, and
follow all outlined rules:

- Applicant must be a National Member registered with AED National
Office
- An AED chapter may select & submit only one (1) application
for scholarship consideration!
- Your
completed application package must include the following:
FAILURE TO INCLUDE ITEMS 2-5 WILL INVALIDATE YOUR APPLICATION!!!
- This form must be submitted AS IS; any alteration will invalidate
your application!
- Official or unofficial transcript(s)
- Letter of support from your
Chapter Advisor
- Letter of support from ONE - ONLY ONE! - Chapter
Officer
- Applicant must be a graduating Senior and accepted for admission into a Health Professions School
- Color, glossy, wallet size, professional
quality photograph for publication in our Society’s journal, “The
Scalpel”
- IMPORTANT NOTICE!!
Your complete application package MUST BE POSTMARKED no later than May 15, 2010 and mail only one (1) per chapter to the following address:
