AED offers several scholarship
applications for Chapter Member:.
Please read through these guidelines, and
follow all outlined rules:

- Applicant must be a National Member registered with AED National
Office
- An AED chapter may select & submit only one (1) application
for scholarship consideration!
- Your
completed application package must include the following:
FAILURE TO INCLUDE ITEMS 2-5 WILL INVALIDATE YOUR APPLICATION!!!
- This form must be submitted AS IS; any alteration will invalidate
your application!
- Official or unofficial transcript(s)
- Letter of support from your
Chapter Advisor
- Letter of support from ONE—ONLY ONE!—Chapter
Officer
- Color, glossy, wallet size, professional
quality photograph for publication in our Society’s journal, “The
Scalpel”
- IMPORTANT NOTICE!!
Your complete application package MUST BE POSTMARKED
no later than May 15, 2008 and mail only one (1) per chapter to the

Scholarship applicants will be notified of the
evaluation results Fall 2008.
